Abstract Glucocorticoidsremainacornerstoneofguideline--dmanagementofpersistentasthmaandallergic diseases.Glucocorticoid-inducedosteoporosis(GIO)isthemostcommoniatrogeniccauseofsecondaryosteoporosisandanissueofconcernforphysicianstreatingpatientswithinhaledororalglucocorticoidseithercontinuouslyorintermittentlyPatientswithGIOexperiencefragilityfracturesatbetterdual-energyx-rayabsorptiometryT-scoresthanthosewithpostmenopausalorage-relatedosteoporosis.Thismightbeexplained,atleastinpart,bytheeffectsofglucocorticoidsnotonlyonosteoclastsbutalsoonosteoblastsandosteocytes.EffectiveoptionstodetectandmanageGIOexist,andamanagementalgorithmhasbeenpublishedbytheAmericanCollegeofRheumatologytoprovidetreatmentguidanceforclinicians.ThisreviewwillsummarizeGIOepidemiologyandpathophysiologyandassesstheroleofinhaledandoralglucocorticoidsinasthmaticadultsandchildren,withparticularemphasisontheeffectofsuchtherapiesonbonehealth.Lastly,wewillreviewtheAmericanCollegeofRheumatologyGIOguidelinesanddiscussdiagnosticandtherapeuticstrategiestomitigatetheriskofGIOandfragilityfractures.
